Daniel J. Sullivan,
MD, JD, FACEP
President & CEO Dr. Sullivan is President
and CEO of The Sullivan Group. He is board certified
in emergency medicine, a Fellow in the American College
of Emergency Physicians, a member of the American Medical
Association, and an Associate Professor of Emergency
Medicine at Cook County Hospital/Rush Medical College.
He has been practicing emergency medicine for over 25
years. Dr. Sullivan obtained his law degree from IIT
Kent College of Law in 1989.
Dr. Sullivan is the Chairman of the Board of MEA Management,
LLC, providing emergency medicine contract services
to Emergency Departments, Urgent Care Centers, and Occupational
Medicine Facilities throughout the Midwest. He is the
Past Chairman of the Department of Emergency Medicine
at Ingalls Hospital in Harvey, Illinois (1990–2000).
He is a Past President of the Illinois College of Emergency
Physicians and was the Executive Editor of the Emergency
Department Legal Letter from 1996 to 1998.
Dr. Sullivan has authored numerous patient safety and
risk management-related books and articles, including:
- High Risk Acute Care, The Failure to Diagnose, a
web-based educational series for Emergency Physicians,
Allied Health Professionals and Emergency Nurses.
- Co-Editor of Emergency Medicine Risk Management,
2d ED., American College of Emergency Physicians.
- Co-Author of Before You Sign - Contract Basics for
the Emergency Physician, American College of Emergency
Physicians.
- Author of EMTALA, A Comprehensive Review, a text
and web-based educational program.
- Author of numerous web-based educational offerings
focused on emergency medicine error reduction, quality
patient care, risk management, and EMTALA.
Dr. Sullivan is a regular speaker on patient safety, risk
management, EMTALA, and other medical legal topics for
the American College of Emergency Physicians. He has also
been a guest speaker for the American College of Osteopathic
Emergency Physicians and has provided numerous risk management
programs for insurance companies, hospitals, legal firms,
and other organizations.
Dr. Sullivan is an expert on the Emergency Medical Treatment
and Active Labor Act (EMTALA). He has been invited to
speak at many hospitals around the country that have organized
EMTALA programs, and he has assisted numerous hospitals
in response to alleged EMTALA violations. |

Fred Callahan, MD
Physician Risk Consultant:
Neurology Dr. Callahan is a pioneer in the
fields of stroke treatment and integrated vascular care.
Following postgraduate training in internal medicine
at Vanderbilt University School of Medicine, Dr. Callahan
served a residency in neurology at Massachusetts General
Hospital and a fellowship in neurology at Harvard Medical
School. He currently has a private practice in neurology
in Nashville, TN, has served on the clinical faculty
at Vanderbilt, and is Past President of the Nashville
Chapter of the American Heart Association, Dr. Callahan
has written many medical publications, holds three patents,
and has authored The Next Medical Revolution: Angiology.
Dr. Callahan has certifications from the American Board
of Internal Medicine and the American Society of Neuroimaging
(neurosonology-physics and cerebrovascular disease).
Publishing under the name A.S.Callahan III, Dr. Callahan
has an extensive list of writings related to neurological
topics dating from 1975 to the present. He has participated
in PROACT II, a phase 3 clinical trial of intra-arterial
thrombolysis, and is a member of the Steering Committee
for SPARCL (Stroke Prevention by Aggressive Reduction
in Cholesterol Levels), a worldwide trial of atorvastatin
for secondary stroke prevention in patients with no
history of coronary heart disease.
Dr. Callahan has assisted many hospitals and physicians
in the implementation of Acute Stroke Treatment Teams.
He joined The Sullivan Group in 2005 to continue the
effort of bringing acute stroke treatment to every community
in America. |

John S. Carbone, MD, JD, FCLM
Physician Risk Consultant:
Psychiatry
Dr. John Carbone has over 21 years of experience as a practicing mental health provider,
specializing in general and forensic psychiatry. In addition to his leadership role as Chief of
Psychiatry and Mental Health Services for the North Carolina Department of Correction in
Raleigh, he was recently appointed by Governor Beverly Perdue to serve on the Commission
for Mental Health, Developmental Disabilities, and Substance Abuse Services. Before affiliating
with the Department of Corrections, Dr. Carbone practiced widely in direct patient care and in
psychiatric consultation-liaison capacities in both public and private settings, including mental
health clinics, state and community hospitals, multi-specialty offices, and forensic facilities in
Virginia, West Virginia, Tennessee, and North Carolina. Having earned M.D., J.D., and M.B.A.
degrees, Dr. Carbone has extensive experience in medicine as well as expertise in both law and
business. He is admitted to practice law before the State Bar of New Mexico, the U.S. District
Court of New Mexico, and the U.S. Court of Appeals for the Federal Circuit; he is also certified
to conduct mediated settlement conferences pursuant to North Carolina superior court civil
actions. One of his medico-legal publications has been cited in a United States Supreme Court
amicus brief. Dr. Carbone has been a Diplomate of the American Board of Psychiatry and
Neurology since 1995 and a Fellow of the American College of Legal Medicine since 2007. |

John Hafner, MD
Physician Risk Consultant:
Emergency Medicine
Dr. Hafner is a Research and Statistical Consultant for
The Sullivan Group. He is board certified in emergency
medicine, a Fellow in the American College of Emergency
Physicians, a member of the American Medical Association,
and a Clinical Assistant Professor of Surgery - Emergency
Medicine at the University of Illinois College of Medicine
at Peoria. He currently serves as Core Faculty and the
Research Director for the Emergency Medicine Residency
program at the University of Illinois College of Medicine
at Peoria. He graduated from the University of New Mexico
School of Medicine, and completed residency training at
the University of Illinois College of Medicine at Peoria.
He obtained his Master’s of Public Health from the
University of Illinois.
Dr. Hafner is a full-time Attending Physician in the Department
of Emergency Medicine at OSF Saint Francis Medical Center,
a level 1-trauma center in Peoria, Illinois. He is the
Chairman of the Institutional Review Board at the University
of Illinois College of Medicine at Peoria and serves as
Editor-in-Chief of Emergency Medicine Specialty Reports.
In 2006 Dr. Hafner was awarded the Ron W. Lee, MD Excellence
in Pediatric Care Award by the Illinois Department of
Public Health for his efforts in child advocacy and community
injury prevention.
Dr. Hafner has lectured on and published many topics,
including medical error, adverse drug events, evidence
based medicine, and unintentional injury prevention. |

Martin G. Hellman,
MD, FAAP, FACEP
Physician Risk Consultant:
Pediatrics Dr. Martin (Marty) Hellman is board
certified in emergency medicine and pediatrics. Upon
completion of a pediatric residency at St. Christopher’s
Hospital for Children in Philadelphia, he was in private
pediatric practice for four years. He then returned
to medical training with a residency in emergency medicine
in New York. Since then Dr. Hellman has taught and practiced
emergency medicine in multiple hospitals in the northeast
Ohio area, where he won five teaching awards.
During his career Dr. Hellman has lectured extensively
on a variety of pediatric topics relating to the care
of children in Emergency Departments. He’s presented
at the annual American College of Emergency Physicians
Scientific Assembly since 1994 and was named outstanding
speaker in 2001. Dr. Hellman has been one of several
speakers at the nationally acclaimed Ohio Emergency
Medicine Review Course since 1988 and was Chairman of
the course in 1990. He’s also spoken at multiple
EMS courses around the country. Dr. Hellman presently
serves on the Editorial Board of EMS Magazine.
Dr. Hellman has written extensively on many topics in

Jim Hubler, MD, JD,
FACEP, FAAEM, FCLM
Physician Risk Consultant:
Emergency Medicine
Dr. Hubler is a graduate of Southern Illinois University's
MD JD program; a combined six-year law and medicine program.
Dr. Hubler is board certified in emergency medicine (ABEM).
He is a member of the American College of Emergency Physicians’,
Medical Legal Committee. Dr. Hubler is also a member of
both the Illinois State and Peoria County Bar Associations.
Dr. Hubler is a core faculty member of the University
of Illinois College of Medicine’s Emergency Medicine
program and is an Assistant Clinical Professor at the
University of Illinois College in Peoria, Illinois. Dr.
Hubler works clinically at OSF St. Francis Hospital in
Peoria Illinois, a level 1 trauma center and emergency
medicine teaching program of the University of Illinois.
He was Executive Editor of the ED Legal Letter for three
years and has written and lectured extensively on risk
management issues in emergency medicine. He has lectured
nationally on EMTALA – The Emergency Medicine Treatment
and Active Labor Act. Dr. Hubler also is a regular presenter
at the American College of Legal Medicine annual conference
and is a member of the Nominating Committee of the American
College of Legal Medicine, as well as the Graduate Student
Ad Hoc Committee of the American College of Legal Medicine.
Dr. Hubler has worked as a Physician Risk Consultant for
The Sullivan Group for three years. He has taken an active
lead in our EDIS (emergency department information systems)
development. He is also a major contributor to the TSG
Advisory Board. Dr. Hubler has written several courses
for TSG’s computer based training and is responsible
for the monthly and yearly updates of TSG’s on-line
education courses. Dr. Hubler is committed to assisting
others in developing risk and safety programs that will
help reduce medical errors, reduce litigation and save
patient lives. |

Henry Lerner, M.D.
Physician Risk Consultant: Obstetrics and Gynecology
Dr. Henry Lerner graduated from Harvard Medical School where he is Assistant Clinical Professor of Obstetrics & Gynecology. Since completing his residency in obstetrics and gynecology at the University of California, San Francisco, 30 years ago, Dr. Lerner has been in practice in Newton, Massachusetts.
Dr. Lerner has a special interest in the phenomenon of shoulder dystocia and has written and lectured widely on the topic. In addition, Dr. Lerner has extensive experience with medical liability. He has served as a Director of ProMutual, a major medical liability carrier, for 14 years, has participated as an expert witness in hundreds of medical-legal cases, and has spoken and published on problems confronting America's medical liability system. |

Lee Schmidt is a Nurse Attorney with over 25 years of
clinical and legal experience. Lee has focused her career
on education, risk management and patient safety issues.
She is a Past President of the Chicago Area Nurse Attorney
Association (CANAA). She has taught basic clinical nursing
and has recently taught "Legal Nurse Consulting"
for nurses at the College of Lake County.
Lee began her career by receiving her BSN from the College
of St. Teresa in Winona, MN. She received a Master of
Science degree in Public Health Nursing and a Family Nurse
Practitioner certificate from the University of Illinois.
Lee earned her JD degree from IIT Chicago Kent College
of Law. She is the President of Medical Analyst Limited,
a medical legal consulting practice. |

Paul Sierzenski MD, RDMS, FACEP
Physician Risk Consultant: Ultrasound
Paul Sierzenski, MD, RDMS, FACEP, is a diplomat of the American Board of Emergency Medicine, is certified as a Registered Diagnostic Medical Sonographer, and is a Fellow of the American College of Emergency Physicians and the American Academy of Emergency Physicians. He works for Doctors for Emergency Services at Christiana Care Health System where he serves as the Director of Emergency, Trauma, and Critical Care Ultrasound; the Director of the Emergency Ultrasound Fellowship; and lead research faculty for the Department of Emergency Medicine. In 2002, Dr. Sierzenski co-founded Emergency Ultrasound Consultants, LLC (EUC), a national leader in point-of-care ultrasound education and quality assurance solutions. The company includes more than 25 board-certified and fellowship-trained physicians who are nationally certified in ultrasound.
Dr. Sierzenski has lectured nationally and internationally on topics of emergency and bedside ultrasound, imaging, stroke, and quality measures. He has authored over 50 peer-reviewed papers, abstracts and texts on topics in ultrasound, imaging and diagnostic efficiencies. While authoring many of the national policies on the use of ultrasound in acute care, Dr. Sierzenski has represented ACEP on several committees and inter-specialty conferences regarding aspects of healthcare reform in emergency medicine and acute care within the United States. He is a voting member of the Centers for Medicare and Medicaid Services (CMS) and serves on the CMS Medical Evidence Development Clinical Advisory Committee. Dr. Sierzenski has served on the National Quality Forum Steering Committee for the First Outpatient Imaging Efficiency Measures, and has unique expertise in clinical ultrasound. |

Thomas E. Syzek, MD,
FACEP
Physician Risk Consultant:
Emergency Medicine / Primary Care
Dr. Syzek completed his MD degree and residency training
in Ohio. He is a board certified Emergency Physician,
a member of the American Society for Healthcare Risk Management,
and a Fellow in the American College of Emergency Physicians.
After seven years as a Family and Emergency Physician
in the Colorado Mountains, he returned to Ohio where he
has practiced emergency medicine since 1989.
Dr. Syzek is a Physician Risk Consultant for The Sullivan
Group. He serves as Director of Risk Management for Premier
Health Care Services, a regional emergency physician group
practice with more than 350 physicians. He also serves
as President of Pinnacle Medical Protective, SPC, and
a physician liability insurance company in Georgetown,
Grand Cayman.
Dr. Syzek is a frequent author and speaker on the topics
of patient safety, risk reduction, litigation support,
physician group risk management, peer review, and physician
liability insurance. |

Alida M. Zamboni,
RN, MS
Nurse Consultant
Ms. Zamboni has 23 years of nursing experience with
over 21 years in emergency services. With a keen interest
in education, she was responsible for managing the education
program and business administration of the prominent
EMS system in DuPage County, Illinois. The EMSS included
31 provider agencies and 10 associate hospitals. She
went on to the position of Emergency Services and Trauma
Director at Provena Mercy Medical Center in Aurora,
Illinois. She oversaw a complete renovation/rebuild
of the Emergency Department, two successful JCAHO accreditation
surveys, EDAP (Emergency Department Approved for Pediatrics
- Illinois) certification, and the resulting volume
increase from 28,000 to 42,000 visits per year. In the
fall of 2004 she helped coordinate efforts that resulted
in achieving Stroke Center status within a 90-day time
frame. Most recently, she was initiating plans to add
a Clinical Decision Unit and achieve accreditation as
a Chest Pain Center.
Recognized for her organizational ability and enthusiasm
for a worthwhile cause, Ms. Zamboni was asked to be
a co-champion in a very successful patient satisfaction
project at PMMC. Additionally, she directed the Clinical
Education Team for the hospital. She has authored articles
for pre-hospital magazines and textbooks and has lectured
nationally and internationally on emergency and pre-hospital
education topics. |
